Nominations for Premier’s Certificates of Recognition for Outstanding Volunteer Service are open from Monday 3 August to Wednesday 30 September.

- the form will allow one nomination for an individual or a team at a time
- you may submit as many nominations as you like
- the online form cannot be saved to return and complete later, so all fields MUST be completed at the same time
- the volunteer name, team name and organisation names will be printed exactly as entered on the form so please ensure all spelling is correct before submitting
