Program Partners

Whether you are looking for support for yourself, a friend or a family member, or you are a service agency wanting to refer somebody to the program, the Regional Coordinating Partner in your area is your first point of contact.

Regional Coordinating Partners coordinate the program across their region, ensuring that all other partners work together to provide the most appropriate services and support for participants. These partners can work with the participant to find an appropriate service or community group to reach their agreed goal.

Care Partner service changes

From 30 June 2023, Care Partner services will no longer be part of the Community Connections Program and will cease in their current form. Care Partner services will cease accepting new referrals from 1 April 2023. New referrals for people with complex needs unable to be met through mainstream supports should now be referred to the DHS Exceptional Needs Unit (ENU). Carer Support Partner service changes

From 30 June 2023, Carer Support Partners will no longer be part of the Community Connections Program. DHS funded Carer Support Services will focus on providing dedicated support to young carers and carer breaks. As the Commonwealth funded Carer Gateway has matured since its inception in 2019, it has become the primary platform for funding carer support services in South Australia. For carers seeking connection to services and community, CCP Partners will continue to provide services to eligible unpaid carers as one of the CCP’s Priority Access Groups. Aboriginal Community Partners provide a range of culturally appropriate services to support Aboriginal peoples to increase their independence, improve quality of life and enhance their connections with communities, social networks, culture and services.
