This informative session will help you understand Advance Care Directives (ACDs) and how they can support your future healthcare, lifestyle and end-of-life decisions.
Facilitated by trained aged care staff, you'll learn how an ACD can help ensure your voice is heard if you are ever unable to make or communicate decisions due to illness or injury.
There will also be an opportunity to ask questions and gain practical guidance on planning ahead.
